What this service covers
Patient engagement means a patient is receiving communication. Patient activation means they have the knowledge, skills and confidence to make decisions about their own health and care. The two are not the same, and most programs stop at engagement.
We design programs that move patients from passive recipients of information to active participants in their own care. This includes patient profiling, education delivery, communication design, goal setting and adherence support.
The programs are built around the realities of each patient population: health literacy levels, condition complexity, cultural context and the capacity of the service to deliver.
What activation looks like in practice
An activated patient understands their condition, knows what they need to do, and has the confidence to do it. They ask questions. They follow through on treatment plans. They make informed decisions about lifestyle, medication and self-care.
This does not happen by sending a brochure or an appointment reminder. It requires structured education, tailored communication and ongoing support that meets patients where they are.
We use validated frameworks for measuring patient activation, including the Patient Activation Measure (PAM), to assess baseline levels and track progress over time.
